PHOENIX, AZ – According to 12 News, officials have released the name of the woman who fatally collided with a Phoenix city bus near N 19th Ave and W Cactus Rd on Monday.
Melody Stewart, 31, was heading north on 19th Ave when she strayed into the southbound lanes and hit the bus head-on around 9 a.m.
Rescue crews performed life-saving measures on Stewart before pronouncing her dead at the scene. They treated three bus passengers and transported one of them to an area hospital with non-life-threatening injuries.
The Phoenix Police Department is investigating the incident.
